At HCMS, we support an individual's decision to stay in their home.  That's why we make the house calls instead of you going into a clinic. We coordinate with community resources to support this choice.  When staying at home is no longer a reality, we can follow our patients into their new assisted living environment to provide continuity of care.

  Our providers are different than the home health and hospice personnel that may be part of your care team.  We will continue to serve as your primary care provider and make house calls or telemedicine service, and coordinate care among agencies.   We work in collaboration with your other physicians or clinics to provide the highest level of care.
